Calm Path Health Navigation offers patient navigation services led by RNs, focusing on non-clinical health education and care transitions support for individuals and caregivers who feel overwhelmed by health information, care transitions, or complex systems. Our services are designed to complement—not replace—clinical care, enhancing patient understanding, follow-through, and confidence between healthcare visits.

Documentation is client-held unless consent is provided, aligning with our commitment to non-clinical health education. Services are delivered within the scope of Registered Nursing practice.
