Terms and Conditions

Effective Date: January 1, 2026


By engaging with Calm Path Health Navigation, you agree to the following Terms & Conditions. Please read them carefully before using our services.


1. Nature of Services

Calm Path Health Navigation provides RN-led educational, consultative, and health navigation services. Services are designed to support understanding, decision-making, and care planning.

Services do not include:

  • Medical diagnosis
     
  • Prescribing medications
     
  • Ordering or interpreting diagnostic tests
     
  • Providing medical treatment
     

Services are not a substitute for care provided by a physician, nurse practitioner, or other regulated healthcare provider.


2. Scope of Practice

All services are delivered within the professional scope of practice of a Registered Nurse in Ontario, in accordance with the College of Nurses of Ontario (CNO) standards.

Clients remain responsible for:

  • Making their own healthcare decisions
     
  • Following up with their primary care provider or specialist
     
  • Seeking urgent or emergency medical care when needed
  •  

3. Client Responsibilities

By using our services, you agree to:

  • Provide accurate and complete information
     
  • Inform us of any significant changes to your situation
     
  • Use the information provided as educational guidance only
     

Calm Path Health Navigation is not responsible for outcomes related to decisions made by clients or families based on the information provided.


4. Privacy & Confidentiality (PHIPA – Ontario)

Calm Path Health Navigation complies with the Personal Health Information Protection Act, 2004 (PHIPA) of Ontario.

Collection of Personal Health Information

We may collect personal health information (PHI) as defined under PHIPA when it is necessary to provide consultative or educational services. This may include information about health conditions, care needs, medications, and caregiving situations.

Use of Information

Personal health information is used solely for:

  • Providing requested services
     
  • Communicating with clients
     
  • Maintaining professional service records
     

PHI is not used for diagnosis, treatment, or clinical decision-making.

Disclosure of Information

Personal health information will not be disclosed without consent except:

  • As required or permitted by law
     
  • Where there is a risk of serious bodily harm
     
  • As required for regulatory or legal obligations
     

5. Storage & Safeguards

Personal information and personal health information are stored securely using password-protected systems and reputable digital platforms.

Reasonable administrative, technical, and physical safeguards are used to protect information from:

  • Unauthorized access
     
  • Loss
     
  • Disclosure
     
  • Misuse
     

Records are retained only for as long as necessary to fulfill service purposes or as required by law.


6. Access, Correction & Withdrawal of Consent

In accordance with PHIPA, clients have the right to:

  • Request access to their personal information
     
  • Request corrections to inaccurate information
     
  • Withdraw consent for the collection or use of information (subject to legal limitations)
     

Requests must be made in writing and will be addressed within a reasonable timeframe.


7. Electronic Communication & Virtual Services

Clients acknowledge that electronic communication (email, virtual meetings, online forms) carries inherent privacy risks.

By engaging in virtual services, you consent to:

  • The use of electronic communication
     
  • The associated privacy risks
     

Efforts are made to use secure platforms; however, complete confidentiality cannot be guaranteed.


8. Payment & Cancellations

  • Fees are due prior to service delivery unless otherwise agreed
     
  • Packages must be used within the specified timeframe
     
  • Cancellations require 24 hours’ notice
     
  • Missed or late cancellations may be subject to full fees
     

Refunds are not provided for services already rendered.


9. Limitation of Liability

Calm Path Health Navigation is not liable for:

  • Medical outcomes
     
  • Decisions made by clients or families
     
  • Actions taken or not taken based on educational guidance
     

Services are provided “as is”, without guarantees or warranties.


10. Changes to Terms & Conditions

These Terms & Conditions may be updated periodically to reflect changes in services or legal requirements. The most current version will be made available upon request or on our website.
